1. By Owner's/Shipper's signature or Owner's/Shipper's
agent's signature. Carrier and Driver or Agents, jointly and separately are authorized to operate and transport the vehicles(s)
from the point of origin specified to point of departure for service, and the final specified destination as set forth in
this Shipping Order. Carrier is authorized to drive said vehicle for pickup/delivery purposes and insurance for same will
be provided by Owner. 2. Roadside Auto and Truck Transport agrees to provide a carrier to transport your vehicle(s) as
promtly as possible in accordance with your instructions but does not guarantee pick-up and delivery dates or in-transit times.
All dates and assertions made by Roadside Auto and Truck Transport are the best estimates available at the time the assertion
was made. As such Roadside auto and Truck Transport will not honor any auto rental accrual under any circumstances, nor shall
it be liable for failure of mechanical or operating parts of your vehicle. 3. A $200.00 administration fee is charged
unless the order is cancelled more than seven days before first requested pick-up date. 4. All oversided vehicles such
as recreational vehicles, SUVs, trucks, and the like, which take up more than normal space on a truck, will be charged an
additional fee. Such fee will be added onto the amount to be collected should it be discovered that the vehile(s) is oversized
and was not quoted initially. 5. All non-operational vehicles tendered for transportation will be charged an additional
$150.00. If a vehicle is tendered for shipment and later becomes non-operational, the $150.00 fee will be added on to the
amount to be collected, as well as any other charges necessary to acomplish delivery. Owner/Shipper or Owner's/Shipper's agents
will make arrangements with transport carrier to meet the driver to load and/or unload, at a safe and legal place if weight
restrictions, low trees, small roads and numerous other obstacles make it impossibe and/or impracical to pick-up or deliver
at the door. Owner/Shipper may be required to pick up vehicle from carrier's terminal if additional fees must be paid on delivery
or if transport carrier is unable to deliver for any reason, and pay any terminal fees, if applicable, in addition to the
regular delivery fees. 6. All fees not paid when order is tendered for shipment will be added on to the amont to be paid
on delivery. All deliveries will be pain in cash, cashier's check or credit card, as no other form of payment will be accepted.
7. By Owner's/Shippers's signature or Owner's/Shippers's agent's signature on this shipping order, the persons listed
at the point of origin and point of desination are appointed agents for the purposes of releasing and/or accepting delivery
of vehicle, approving charges, and releasing Roadside Auto and Truck Transport and Driver from any and all claims upon acceptance
of delivery. 8. If there is any problem regarding a trucking delivery, Owner/Shipper agrees to pay balance due at the
time of delivery regardless of such problems. Such payments shall be without prejudice to Owner's/Shipper's other remedies
specified herein. Then a copy of the damage report should be sent to the transport carrier immediately, so as to expedite
a proper resolve. Payment to the Driver, or signing the Bill of Lading at destination, without immediate notation of damage,
shall be evidence of satisfactory delivery of vehicle. 9. The Price quoted is for cash payment, (customers using a credit
card will be charged a 3.5% bank service charge.) Pick-up and delivery is from terminals or commercial loading zones in your
area, or from your door unless residential restrictions apply. Pick-ups by appointment are available for an additional fee,
by smaller unrestricted carriers. If Owner/Shipper is not available at delivery, vehicle will be delivered to a terminal,
and there will be an extra charge for storage and delivery. If your vehicle is inoperable or oversize (longer than 16 feet,
dual wheels, large pick-up, van, racks, or weight in excess of 5000 lbs, etc...), please inquire as to extra charge. If carrier
is not advised of inoperable or oversized vehicles prior to pick-up, all extra charges must be paid in cash, money order,
or certified check on delivery. 10. Owner/Shipper shall remove all non-permanent outside mounted luggage and orher racks
prior to shipment. Vehicles must be tendered to carrier in good running condition with no more than a quarter tank of fuel.
Carrier will not be liable for damage caused by leaking fluids, freezing, exhaust systems, or antennas not tied down. Any
claim for loss or damage must be noted and signed on condition report at the time of delivery. 11. LUGGAGE and personal
property must be confined to trunk, with no heavy articles, and not to exceed 100lbs. total. Carrier is not liable for personal
items left in vehicle, or for damage caused to vehicle from excessive or improper loading of personal items. Any article left
in vehicle will be removed at Owner's/Shipper's expense. 12. The following items are not to be in the vehicle at time
of shipment: explosives; guns' ammunition; flammable products; narcotics; negotiable and legal papers; alcoholic bevarages;
jewelery; furs; money; other articles of unusual value; live plants; live pets or contraband. Owner/Shipper agrees that carrier
may confiscate and/or dispose of said items with no remuneration. No other personal property may be left in the vehicle without
authorization from Roadside Auto and Truck Transport prior to pick-up. 13. Trucking damage claims are covered by carrier
for up to $350,000.00 cargo insurance per load, and a $2,000,000.00 dollar public liability and property damage policy. All
claims must be noted and signed for at time of delivery, and submitted in writing within 15 days of delivery. 14. Owner/Shipper
warrants that he will pay the price quoted due Roadside Auto and Truck Transport for delivered vehicles, and will not seed
to charge back a credit card or stop payment on a check to offset any dispute for damage claims or delays. Owner/ SHipper
agrees to pay all collection costs, legal and court fees incurred for the collection of any unpaid amounts owing to Roadside
Auto and Truck Transport. Department of Transportation regulations require that all claims be filed in writing, and all tariffs
be paid in full before claims may be processed. 15. This agreement supersedes all prior written or oral agreements and
understandings between Roadside Auto and Truck Transport and Owner/Shipper and constitutes the entire agreementbetween Roadside
Auto and Truck transport and Owner/Shipper. This Agreement may not be modified except in writing signed by an offiecer of
Roadside Auto and Truck Transport.
